Reports to: Manager, FC CAN

Status: Exempt, Full-Time

Location: Hybrid, Bridgeport

Salary Range: $60,000 - $70,000

Role Overview

The Program Coordinator supports the planning, execution, and monitoring of projects to ensure deliverables are completed on time, within scope, and aligned with organizational goals. This role provides coordination, communication, and administrative support across project teams and stakeholders.

As part of HC’s core infrastructure, the role strengthens internal collaboration, supports smooth information flow between teams and partners, and builds systems and workflows that promote consistency, sustainability, and shared learning, all essential components of effective collective impact backbone work. This is achieved through the development and maintenance of work plans and timelines, facilitation of communication across internal teams and external partners, and translation of strategic direction into clear, actionable steps.

 

Key Responsibilities

  1. Coordinate project activities, timelines, and milestones across multiple workstreams
  2. Track project progress, deadlines, and deliverables; flag risks or delays
  3. Support project planning, including schedules, work plans, and documentation
  4. Facilitate communication between project managers, team members, and stakeholders
  5. Prepare meeting agendas, take notes, and track action items
  6. Maintain accurate project records, reports, and data systems
  7. Assist with resource coordination, logistics, and vendor or partner engagement
  8. Support reporting, evaluation, and continuous improvement efforts
  9. Ensure projects follow established processes, policies, and standards

Specialization: Housing & Exits System

Scope of Work:

The Program Coordinator, Exit will coordinate the housing process and exits within the identified sub-community in the Fairfield County Coordinated Access Network (FC CAN).

  • Reviewing documentation in a timely manner, maintain a high quality of documentation, and collaborate with CAN partners on data quality, as necessary.
  • Submit homeless referrals to housing providers, subsidy holders, and case managers to fill vacancies in a timely manner, and provide required updates and documentation, when requested, immediately to prevent system backlog.
  • Complete CE events, document and update Smartsheets/ HMIS of all housing updates and maintain accurate tracking of housing exits and system flow.
  • Actively attend all housing meetings (housing solution, RRH, case conferencing, PSH subgroup, higher level of care, etc) meetings in each sub-community, prepare agendas that are provided to the network in advance, and send meeting meetings with follow up action steps.
  • Ensure Smartsheet accuracy on all HC ‘owned’ pages including case conferencing, first come first serve, housing vacancy report, and BNL 2.0
  • Provide TA to the network through a monthly CAN 101 training.

 

Additional Qualifications

  • Ability to work in a collaborative, fast paced environment.
  • Knowledge of, or ability to quickly develop an understanding of homelessness, effective intervention techniques, and mental health and substance use issues and interventions.
  • Ability to work effectively with populations that have experienced trauma, deep poverty, incarceration, substance use, compromised mental wellness, literacy and numeracy issues, and other conditions that negatively impact individuals.
